Course Description

Home health is rapidly evolving under the pressures of value-based care, workforce complexity, and rising expectations for quality outcomes and patient experience. This session explores how targeted clinician education can serve as a powerful lever for change in home health practice. Drawing from a doctoral project focused on occupational therapy in home health, the presentation highlights the development and outcomes of a four-part educational model designed to strengthen practitioner self-efficacy, deepen evidence-informed practice, and connect clinical decision-making to organizational performance. Attendees will examine how topics such as technology, population health, medication management, and evidence-informed practice can equip clinicians to work at the top of their license while advancing patient outcomes, reducing risk, and supporting value-based care priorities. This session is ideal for leaders, educators, and clinicians seeking practical strategies to build capacity, elevate practice, and drive meaningful change in the home health setting.
